Agency/Broker Appointment Process
Prospective agencies wishing to access Phoenix's products and programs will need to be appointed. To be considered for an appointment with Phoenix please:
- Complete, sign, and submit Phoenix's Agency Appointment Profile and Broker-Company Agreement
Please include the following items:
- Copy of current agency insurance license
- Copy of in force Errors and Omissions Liability policy dec page or Certificate of Insurance
- Copy of W-9 - Request for Taxpayer Identification Number and Certification

Marketing department will direct and be responsible for agency appointment process which will include the following:
- Interview prospect agencies either by phone or in person
- Oversee completion of all items listed above
- Instruct/provide copies of "Doing Business with Phoenix" (Broker Kit)
- Maintain Suspense file on agencies license, and E & O insurance.
An agency may only be approved by Phoenix's Agency/Broker Appointment Committee which will be made up of the following individuals:
- Vice President of Marketing or designee
- Vice President Underwriting, workers' compensation or designee
- Vice President Package Program or designee
- CFO or designee
Note: Agency Bill consideration requires audited financials
Agency / Broker Review Criteria
All agencies must meet and maintain agency performance requirements to retain appointment. Agency performance reviews will be conducted annually by Phoenix's Agency/Broker Appointment Committee. A review may be called to handle major broker issues. The following items will be reviewed for issues or concerns:
- Agency/Broker support to Phoenix programs
- Collection and invoicing issues
- Losses by Agency
- Procedure/policy issues (ease of doing business)
- Volume by agency (meeting performance requirements which is In-force premium from various Phoenix products in first twelve (12) months after appoitnment.)
